中文摘要: 目的:通过挖掘单位时间内气象因素与成人社区获得性肺炎中医证型发病的关系,为季节性疾病的预测提供启发.方法:收集2009年至2013年广东省中医院成人社区获得性肺炎确诊病例及广东省广州市气象资料,采用LASSO算法(一种线性回归)建立预测模型,采用均方误差作为评价标准.结果:研究共纳入16737例社区获得性肺炎病例,其中风热犯肺证5741例,痰热壅肺证4910例,风热犯肺组采用前8个月的全部气象数据得到的均方误差值为0.4613,相关气象因素为前2个月平均日照时数.痰热壅肺组采用前8个月的全部气象数据得到的均方误差值为0.3393,相关气象因素为前2、3、5、8个月平均日照时数、前3个月平均风速、前7个月平均气温、前7个月平均相对湿度、前8个月平均降水量.结论:气象因素影响成人社区获得性肺炎中医证型的发生,成人社区获得性肺炎的预防和控制及相关公共卫生政策的制定应更多考虑当地气候变化的影响.
英文摘要: Objective: To find out climate-related risk factors and their roles in Community-acquired Pneumonia with syndrome of Chinese medicine. Methods: A database is integrated monthly climate factors and cases of Guangdong Hospital of TCM from 2009 to 2013. Monthly climate data( 2009-2013) were collected from the Guangdong Meteorological Service. One popular algorithm,The Least Absolute Shrinkage and Selectionator operator( lasso), a shrinkage and selection method for linear regression is applied to select related climate factors and build prediction model. Mean square error( MSE) is used as evaluation criterion. Results: There were 16737 cases included 5741 cases which were community-acquired pneumonia with syndrome of wind-heat invading lung and 4910 cases which were community-acquired pneumonia with syndrome of phlegm-heat congesting lung in total from 2009 to 2013. The MSE of community-acquired pneumonia with syndrome of wind-heat invading lung is lowest when we using eight months'climate data. The MSE is 0.4613. Monthly mean sunshine duration of one month before( MSD2) have a strong correlations with community-acquired pneumonia with syndrome of wind-heat invading lung. The MSE of community-acquired pneumonia with syndrome of phlegm-heat congesting lung is lowest when we using eight months'climate data. The MSE is 0.3393. Monthly mean sunshine duration of one month before( MSD2), monthly mean sunshine duration of two month before( MSD3), monthly mean sunshine duration of four month before( MSD5), monthly mean sunshine duration of seven month before( MSD8), monthly mean wind velocity of two month before( MWV3), monthly mean temperature of six month before( MT7), monthly mean relative humidity of six month before( MinRH7), monthly accumulative precipitation of seven month before( AP8) have a strong correlations with community-acquired pneumonia with syndrome of phlegm-heat congesting lung. Conclusion: Climate factors have affected the transmission of community-acquired pneumonia with syndrome of Chinese medicine. Community-acquired pneumonia prevention and control would benefit from by giving more consideration to local climate variations. Relevant public health strategies should be developed at an earlier stage to prevent and reduce the impact of infectious disease associated with climate change.
